Home Health QAPI Self-Assessment Tool

Under the new Conditions of Participation (CoPs), home health agencies will be required to have comprehensive quality assurance and performance improvement (QAPI) programs. Your team needs the right to tools to create a dynamic plan tailored to your agency.

As part of our CoPs webinar series, we’ve adapted a QAPI worksheet from CMS for your team to evaluate and solidify your agency’s program. Use this tool to review your QAPI plan and identify strengths and areas for continued improvement. This worksheet is ready to print and use now.

She has over twenty years experience in home health and hospice settings as an RN with advancing levels of leadership, including the start-up and growth of her own hospice in Southeastern Louisiana. Her certification and specialty are in hospice operations, both clinical and financial, having received several awards and recognitions for management and operational efficiency and excellence through the years.
